Senior Services

At Jewish Family Service our goal is to enable the elderly to remain living independently and with dignity in their own homes. We offer a mix of professional and volunteer services along with concrete services such as transportation, meals and in home programs to accomplish this.

Join the Blue Stars

Join the Blue Stars, a singles group 60 and older, sponsored by Jewish Family Service. Activites, luncheons, informative speakers, theatre trips, Jewish Holiday celebrations and much more.

Services to Seniors include:
	Case Management — We make sure that our seniors have adequate food, clothing and shelter plus contact with the community.
	Individual and Family Counseling on such issues as loss, depression, aging and future planning.
	Meal Programs — Daily Hot Kosher Lunch and Kosher Meals on Wheels for those not able to get out.
	Transportation — To Daily Lunch Program and weekly medical appointments..
	Volunteer Visits, Telephone Calls and Drivers.
	Companion and Homemaker Referral — We have relationships with reliable community agencies that can provide you with your needed services.
	Advocacy
	Caregiver Service — Educational and emotional support is available to caregivers of seniors including monthly support group meetings.
	Geriatric Care Management JFS Geriatric Care Management is an hourly fee-for-service program specializing in the following: Patient assessment and coordination of services Screen, arrange and monitor in-home help or other services Weekly visits and regular phone calls Offer counseling for caregivers and family members Assistance in arranging legal, financial and medical services Provide consumer education and advocacy Act as liaison to families at a distance Review financial, legal or medical issues and offer referrals to geriatric specialists to avoid future problems and conserve assets
	Other services include:
	Caregiver Support Group Jewish Family Service facilitates a Caregivers Support Group for family members or friends helping with the care of adults over 60. The group meets on the first Wednesday of the month from 6:00-7:00 PM at the Jewish Family Service office at 2370 Park Avenue , Bridgeport . February’s Support Group will meet on Wednesday, February 1, 2006 . Please call Joan E. Farley, Elderly Services and Caregiver Resources Coordinator for information at 366-5438.

Meet Sadie

The family says they now have peace of mind.

Sadie is 87 years old. She has been a widow for the last 13 years. She has family, a grown daughter, grandchildren and great-grandchildren, but they live almost 2000 miles away. The last time they visited Sadie, they were concerned about her ability to care for herself. They brought her in to Jewish Family Service to speak to our gerontologist. The agreed to the following plan:
Jewish Family Service helped them find a paid companion. She helps Sadie by preparing meals, cleaning the house and assists with Sadie's personal needs.

A JFS volunteer friendly visitor named Joanne was found. Joanne, and her three-year-old child, now visit Sadie weekly. You should see Sadie light up when Joanne and her daughter arrive! She so looks forward to their company!

Lastly, the Jewish Family Service gerontologist agreed to visit Sadie every other week. She makes sure that Sadie is O.K. She also gives Sadie the opportunity to talk about her concerns.

Meet Joseph

He is an 87-year-old widower. Since his wife died, he didn't get out much. He also lost a lot of weight. His cooking was far from gourmet!

Jewish Family Service received a call from a concerned neighbor and sent a social worker out to visit Joseph. After a few weeks of developing a trusting relationship, Joseph agreed to try riding the mini bus to the daily Hot Kosher lunch program. Joseph liked the program! He met some friends there that he hadn't seen for over 30 years. Joseph now attends the program three days a week. He looks forward to his time out of the house and has even gained a little weight.

Now that Joseph was used to getting out a little more, he agreed to attend a monthly support group for widowers. This has given him opportunity to talk with other men that are in the same situation as he is.

Meet Lee

Lee is a son in New Jersey with a Mother in Bridgeport. Lee's mother was in a skilled care facility, wanting to return home. When he came to the JFS office for assistance to accomplish this goal, he was not aware of the professional Geriatric Care Management Program that the agency offerred.

In working with the Caregiver Resource Coordinator, by the end of that day he and his brother (also out of state) were in touch with the GCM (Geriatric Care Manager). After the GCM's assessment and her recommendations, his mother returned home with all the necessary in home services in place, under the Title 19 funding process.

The family members were relieved and grateful for the professionalism in this difficult health care system and the mother was able to return home, the best situation in this very difficult time.
